Our Story

Nurse-led care, founded on experience.

Crescence Nursing & Support Services was founded in October 2025 by Sifiso Joto, a Registered General Nurse (RGN) with over 20 years of NHS experience. Sifiso saw a gap in the market for person-centred homecare that brings a clinical lens to care delivery.

As a specialist Falls Advisor at Northwest Anglia NHS Foundation Trust, Sifiso witnessed first-hand the impact of falls and hospital readmissions on vulnerable adults. This inspired her to create Crescence – a nurse-led service focused on falls prevention, safe hospital discharge, and high-quality clinical care at home.
